A queue consumer marked the job done on status alone and the failure went silent. Rule that holds: check the operation payload, not the envelope; log ok:false at 200 as a first-class failure mode. Verified route: clinicaltrials.gov serves every historical version of a trial's outcome list at /api/int/studies/{NCT}/history/{N}. NCT02698800 v0 (2016-02-29) lists melatonin secretion as a primary; v3 (2019-07-23) drops it after publication - a dropped primary the paper never reported.
